Informed Consent

You are being invited to participate in a research survey conducted by TheCHANGEproject, a nonprofit organization focused on health equity.

Purpose: This survey collects information about community health needs and barriers to health equity. Results will be used to inform program design and policy advocacy.

What we collect: Demographic information (zip code, age range, race/ethnicity), health access experiences, and your perspectives on community health challenges. No clinical or medical record data is collected.

Anonymity: Your responses are anonymous. We do not collect names or identifying information unless you voluntarily provide an email address. IP addresses are not stored.

Risks: There are no known risks to participating. You may skip any question you are not comfortable answering.

Voluntary participation: Your participation is entirely voluntary. You may stop at any time without penalty.

Data handling: Responses are stored securely and encrypted at rest. De-identified aggregate data may be shared in published reports. Individual responses are retained for 365 days and then permanently deleted.
