Before you start

This is a community feedback survey from TheCHANGEproject, a nonprofit working on health equity in Colorado. It is not a research study and has not been reviewed by an institutional review board (IRB).

What we ask: your zip code, age range, how you identify, your access to health care and insurance, and your views on health in your community. We do not ask for your name. The question about how you identify can be skipped, the insurance question has a "Prefer not to say" option, and you can stop at any time without anything being saved.

How we use it: to decide where to focus our programs and advocacy. We may publish combined results, such as percentages. We will never publish an individual's answers.

Your email: giving it is optional. If you do, it is stored in a separate record from your answers, linked only by your reference code, and used only to contact you about our work.

Who can see it: a small number of TheCHANGEproject staff. Answers are stored with our database provider, which encrypts data at rest.

Deletion: answers and any email you give are deleted automatically 365 days after you submit. You can delete them sooner, at any time, with the reference code shown when you finish.

IP addresses: we don't save your IP address with your answers. Like most websites, our hosting provider keeps short-lived server logs that can include it; see our privacy policy.
